How much will it Cost?

This service will only cost ZMW 120 per annum which works out to only ZMW 10 per month. The Insurance Premium may be paid upfront at a fee of ZMW 120 to cover an entire 12 Months or paid monthly at ZMW 10 deductible from your savings and remitted to the insurance company.

A funeral grant of ZMW 5,000 upon the unfortunate death of your spouse. In the unfortunate event of the main policy holder’s death, a sum of ZMW 10,000.00 will be paid out to your next of kin.

What is the Scope of the Cover?

Death arising from any cause (Suicide within 24 months of effecting the Scheme is not covered)

What is the claim processing turnaround time?

Twenty-Four (24) Hours Only

What do I need to qualify

• Download Application Form or Apply Online
• Copy of ID (NRC or passport or valid driver's licence)
• Submit duly filled in form with copy of ID to customerservice@carecoop.co.zm or visit our office

What do I need for my claim to be processed?

• Certified copy of ID for member and deceased 
• Certified copy of: death certificate or medical certificate of thecause of death and/or a police report in the event of unnatural death
• A letter indicating the death of the member or spouse

SUBMISSION TIMELINE: CareCoop to be informed within One (1) month of the event and documents presented within Six (6) Months
